Following the filing of this application, the Division conducted a duly noticed public hearing in the Divisionþs hearing room, 100 Orange Street, Providence, Rhode Island. From a review of the record in this proceeding, it appears the Applicant named above is fit, willing, and able to provide the services requested in the application. Testimony elicited at the hearing demonstrates that the Applicant has the necessary equipment, experience, and financial responsibility to provide the proposed transportation services. Accordingly, it is ( ) ORDERED: That the Applicant listed above, having applied for authority to transport the item(s) listed above, between points in Rhode Island, is hereby approved subject to the following terms: 1. The Applicant must hand deliver to the Division proof of financial responsibility (liability and cargo) from an insurance company ($10.00 filing fee). 2. All vehicles intended to be used in the operation of the business must be registered with this office ($20.00 fee per vehicle). The company's business address will remain at the same location as listed on the application unless otherwise approved and authorized by the Division. If the conditions contained in paragraphs one and two are not met within sixty (60) days from the date of this order, the approval herein granted will be of no further force and effect. Upon satisfactory completion of the terms and conditions contained in the above-mentioned paragraphs, a certificate of operating authority shall be issued. No transportation services shall be provided until such time as said certificate is issued.
